The presiding authority (mission president) would have to authorize it, and the paperwork would have to be completed and submitted. The person would be assigned to some type of unit, even if the ward or branch meets relatively far away. There used to be a "worldwide" unit (it may have been designated a mission), to encompass all those who lived in remote locations, in countries where the church did not have a presence, etc. It is not easy to fellowship in those circumstances, but communication can be made, and materials can be ordered.

As long as the mission president authorizes it (he has the priesthood keys), then this good lady can be baptized officially and have it recognized both on earth and in heaven. No need to do anything illegal or unauthorized. The Lord has a plan and a way to put things in proper order. It doesn't matter if there are no local congregations to worship with. She can be a full member in good standing, even if she would be the only member in her area.
