It means that you have and exercise faith that Jesus' atonement can cleanse you. This is done by faith, and repentance, and baptism. I think a good way to exercise faith and repentance is confessing your sins in prayer to Father in Heaven, and asking Father in Heaven to apply the blood of Christ to cleans your sins. There may be other people you need to speak with also to fully repent (such as a bishop and also those you have offended, to be reconciled with them if applicable). If you have already been baptized, then it is a continuous cycle of repentance and asking for forgiveness, and increasing faith in Jesus and the atonement. Taking the sacrament is the priesthood ordinance wherein we witness to the Father that we are doing these things - we are willing to take upon ourselves the name of Christ, always remember Him, and keep his commandments. This is how we apply the atoning blood of Christ. The scriptures also use other terms for this, such as washing our garments in the blood of Christ so that they may become clean.
